We process all applications on a first come, first served basis. Once you have completed your application, you can choose one of three ways to return it to us:

1. scan it and email it to rentals@newhorizonwi.com (instant return)

2. fax it to 414.797.4326 (fast return)

3. mail it to P.O. Box 240044, Milwaukee, WI 53224 (slowest return)

PROVIDING FRAUDULENT INFORMATION TO OBTAIN A FEDERAL HOUSING BENEFIT CAN LEAD TO INELIGIBILITY FOR HOUSING, PROSECUTION, FINES, AND POSSIBLE JAIL TIME!

You need to complete and return both the Application and HUD Consent Form for your application to be complete.
