Application, Forms and Additional Information
The New Horizon 2024 Application and the HUD Consent Form are both needed to apply for housing. Make sure they are COMPLETE, SIGNED, AND DATED or your application will be rejected.
You are applying for Federally funded housing with the U.S. Department of Housing and Urban Development. Please read the HUD Fraud Warning for providing fraudulent information on your application for low-income housing.
Please print out the forms you need. You can scan and email them to rentals@newhorizonwi.com or you can fax them to 414.797.4326. You can also send them via USPS mail, but this is the slowest way to get your forms to us. Applications are processed and apartments assigned on a first-come, first-served basis.

NEW HORIZON NO SMOKING POLICY
ALL our HUD properties are SMOKE-FREE. There are penalties if you smoke ANYTHING inside our buildings. You are only permitted to smoke 25 feet away from the building, and windows and doors.
NEW HORIZON PET POLICY
We allow pets in our properties. Your pet must have all its shots, be registered with us, and the pet deposit must be paid BEFORE you can move in your pet.
NEW HORIZON BANK ASSET FORM
We are required to verify the sources and amounts of all income of applicants. This form must be completed by your financial institution. Call our office for ways to verify your financial assets if you do not have a bank account.
NEW HORIZON DISABILITY VERIFICATION FORM
We are required to verify disabilities to award deductions or qualify individuals for disabled housing. This form must be completed by your healthcare professional. They will send it directly to our office for you.
